Please note that a text message will be sent to all new patients the morning of your scheduled appointment.  This text will contain a link where you can complete all new patient questions online, through a secured website.  

If you are unable to complete the electronic questionnaire for any reason, please print and complete the new paitent forms in the link below and email the completed forms to info@parkwayprimary.  Please note that there is a covid screening questionnaire that must be answered on the same day as your scheduled appointment.  Let us know if you have any questions regarding this.
